INITIAL EVALUATION (TELEHEALTH)

Service Description

Choose this option for a formal evaluation of your current pain or problem area. During this private, one-on-one video session, the physical therapist will evaluate your current condition and give individual treatment recommendations.

Cancellation Policy

All cancellations must be made 24 hours prior to my appointment. I understand that I may incur cancellation/no show fees up to 100% of the session if I do not show up for my appointment or fail to cancel within 24 hours
